Teacher Feedback Form for Science Fairs, Career Fairs and Special Events

Your feedback is essential to the running of our programs and helps ASN to continue to offer them at no cost to schools.

It also helps us to:

  • monitor how the program is running
  • find ways to improve
  • mentor and provide feedback to our volunteers
  • collect statistics for grant applications

Please complete the following short questionnaire and provide as much detail as you can. Consider sending a thank you note or letters from your students to the scientist/engineer volunteer. Our volunteers tell us that they appreciate thank-yous from students more than anything else.
